INFORMATION FOR PATIENTS REGARDING WUHAN NOVEL CORONAVIRUS INFECTION

All travellers who develop relevant symptoms (for example, respiratory symptoms, shortness of breath, cough, fever) however mild, within 14 days of returning or transitioning from mainland China, Thailand, Japan, Repbulic of Korea, Hong Kong, Taiwan, Singapore, Malaysia or Macau should not attend the practice, but should remain at home and immediately call NHS 111 for advice.

Measles vaccines

Due to outbreaks of measles being reported in the South of England we are recommending anyone who has not had 2 doses of the measles vaccine (known as MMR) to make an appointment with our nurse to have this done.  This is especially important if you are travelling to Romania, Italy or Germany.

 

 

 

New Patient Registration

As a new patient you will be required to fill in two forms for each patient registering. These are obtainable from the receptionist and when completed should include important information on current medication, immunisations, allergies and relevant medical family history. For children under 16 you will need to bring in their birth certificate.

Make an appointment either in person or on the telephone with a member of our reception staff. When you arrive for your appointment please advise the receptionist who will inform the clinician, and wait to be called. If you require any special help whilst in the building please advise the receptionist when you make the appointment.
